Can an American licence holder drive in Macau?

Yes. Your US licence is accepted for a visit, and an International Driving Permit is recommended to carry alongside it — the document rental operators and police expect to see with your American licence and passport. The side switch is the catch

Macau was Portuguese for centuries and, like neighbouring Hong Kong, drives on the left — the opposite of the United States. That reversal is the main hazard, and it bites hardest precisely where Macau is most challenging: narrow, crowded, one-way streets where there is no room for a slow reflex. If you are used to right-hand traffic, take extra care at every junction, roundabout and overtake, and consider whether the short distances really justify driving at all.

Alcohol and the practical reality

Macau enforces a 0.05 limit, below the 0.08 of most US states — treat it as zero before driving.
