Yes, you can drive in El Salvador on your own US license: Canadian and UK travel advice both give visitors the first 90 days of their authorised stay on a foreign licence. The surf-and-volcano loop along the coast rewards having your own car, and you already drive on the right. Two things deserve attention before you go: what to do with an International Driving Permit, and a far stricter line on alcohol than at home.

The licence verdict
Carry your US license and passport together. Canada says the foreign licence is valid for the first 90 days authorised for your stay; for longer, you need a Salvadoran licence, which UK advice says requires a temporary residency permit. An International Driving Permit does not help here: UK advice states plainly that El Salvador does not accept the IDP, and Canada suggests one only if you will drive on into neighbouring countries. If your route continues, run it through the driving permit checker.
Alcohol: none at all
Canada’s advice is unambiguous: driving with any detectable amount of alcohol in blood or breath is strictly prohibited, and violators face immediate arrest and prison. A single drink that would leave you legal at 0.08 in most US states can land you in a cell here, so drink nothing before you drive.
Roads and night driving
UK advice says roads between the main tourist locations are of a good or acceptable standard, but lane discipline is poor, and in more isolated places roads are unsurfaced and need a 4x4. Canada lists dangerous curves, poor markings and signage, poor night lighting, potholes and missing manhole covers, and advises avoiding road travel between cities at night.
